Some disasters are caused by fear or misjudgement. When one of the US weapons systems flags an aircraft as a threat, the crew must make quick decisions as the machine gives them mixed signals. Fearing heavy losses, Captain Rogers decides to defend his ship.

Mayday: Air Disaster - from Season 3 Episode 5 " Mistaken Identity": July 3, 1988 - Captain Will Rogers III of the USS Vincennes is stationed in the Persian Gulf. His ship carries one of the smartest weapons system ever designed: The billion dollar Aegis system can track every aircraft within 200 miles – up to a hundred targets at a time - and decide whether they represent a threat.

The USS Vincennes is in the Gulf to help defend traveling oil tankers from hostile Iranian speedboats. Capt. Rogers and his crew are engaged in a sea battle with the speedboats, which are equipped with machine guns and rocket launchers, when the Aegis radar picks up the track of an incoming aircraft. The ship’s sophisticated weapons system has to decide whether the aircraft is friendly, or an attacking warplane. But it can’t seem to make up its mind! The Vincennes crew grow increasingly convinced that the target is an Iranian fighter masquerading as an airliner.
